Positive Practice Proforma

The National Collaborating Centre for Mental Health (NCCMH) and the Positive Practice in Mental Health (PPiMH) Collaborative are working together to help identify and share examples of how good mental health care can be delivered.

The NCCMH develops guidance for NHS England and NICE, including mental health care pathways. For each pathway there is an implementation guide and helpful resources, with examples of good practice. There is more information about the NCCMH and the pathways here.

PPiMH is a user-led multi-agency collaborative of 75 organisations including NHS trusts, CCGs, third sector organisations and service user groups. The PPiMH Collaborative’s aim is to raise the profile of mental health with politicians and policy makers and to disseminate good practice in mental health care. More information about the PPiMH Collaborative on the NCCMH website.

They are inviting you to share with them details about your service or team, especially what you are doing well. By providing us with this information you are eligible for inclusion in the NCCMH’s pathway guidance and on the PPiMH Collaborative’s directory and for consideration for the PPiMH Collaborative’s awards, although the decision will rest with the NCCMH and the PPIMH Collaborative, respectively. By completing this form, you agree to your service information being used by the NCCMH and PPiMH. Any confidential or individual service user related information should not be included on this form.
